Objective
- To integrate clinical and non-clinical data sources by combining principles of Information and Communication Technology (ICTs) to enhance population health outcomes across diverse geographic settings.
- To utilize the PHIK as a tool to assess the burden of chronic diseases and associated risk factors in diverse settings such as urban, urban slums, and rural/tribal settings in India.
Swasthya Pahal ("Health for all"), our community initiative utilizes SMAART™ framework, aims to promote self-management among individuals about diabetes, hypertension, obesity and high cholesterol. Swasthya Pahal utilizes interactive multi-lingual, touch screen computer program that gathers data related to social-demographics, clinical history, and health behaviors.
In addition, objective data gathered includes height, weight, blood pressure, and blood sugar. An evidence based decision rule engine then generates a SMAART™ health card for each individual. Based on the individual risk findings, the system provides an individualized lifestyle feedback supported by trained lifestyle counselling experts.
